CHA PACE provides health care and other support for adults 55 and older who want to continue living at home. We are part of the national Program of All-Inclusive Care for the Elderly (PACE).
With PACE, you get great doctors, rides to medical visits, medications and more – the things you need to live safely in the community.
PACE also covers all the costs of care. You won’t worry about co-pays, pharmacy costs or other costs that may cause financial stress. Our team is here for you.
You may be eligible if you...
- Are 55 years old, or older
- Live in the CHA PACE region: Arlington, Belmont, Cambridge, Charlestown, Chelsea, Everett, Malden, Medford, Revere, Somerville or Watertown, MA
- Have medical needs that qualify you for PACE as determined by the state (call us for more details)
- Can live safely in a community setting with appropriate support
- Agree to receive services from CHA PACE and our authorized network of providers
