Independence, Dignity and Health

CHA PACE provides health care and other support for adults 55 and older who want to continue living at home. We are part of the national Program of All-Inclusive Care for the Elderly (PACE).

With PACE, you get great doctors, rides to medical visits, medications and more – the things you need to live safely in the community.

PACE also covers all the costs of care. You won’t worry about co-pays, pharmacy costs or other costs that may cause financial stress. Our team is here for you.

You may be eligible if you...

  • Are 55 years old, or older
  • Live in the CHA PACE region: Arlington, Belmont, Cambridge, Charlestown, Chelsea, Everett, Malden, Medford, Revere, Somerville or Watertown, MA
  • Have medical needs that qualify you for PACE as determined by the state (call us for more details)
  • Can live safely in a community setting with appropriate support
  • Agree to receive services from CHA PACE and our authorized network of providers
