Helping older adults maintain their health, dignity and independence

We never know when a spouse, parent, or older relative will have a health issue that makes it difficult to maintain their independence.

Sometimes the change is swift, for example with a stroke. But many times our loved ones’ health just seems to be a slow and steady decline.

While there are many services that can help, one little-known program, the Elder Service Plan, is literally transforming the lives of frail and disabled older adults.

Part of the national Program of All-inclusive Care for the Elderly (PACE), the Elder Service Plan offers everything older adults need to maintain their health, wellness and independence. Some highlights include:

  • Health care from an experienced team, including geriatric doctors and nurses.
  • No co-payments or out-of-pocket expenses for medications.
  • Support in a community setting( rather than having to move to a nursing home) tailored to preventing nursing home placement.
  • Companionship and social activities at our centers in Cambridge and Malden.
  • Transportation to and from medical appointments and the PACE centers.
  • Peace of mind for members and their families.

In order to be eligible, adults must be 55 or older, meet the state criteria for the program and want to live at home or in the community (non-nursing home). The CHA program currently covers 13 local communities, recently expanding to Revere and Belmont. But there are PACE programs in other parts of Massachusetts as well.
