MAKING
AN APPOINTMENT WITH A CAMBRIDGE HEALTH ALLIANCE SURGEON
In most cases, you must have approval from your primary
care provider before making an appointment with a surgeon. If you
have a question about the process, please contact your insurance
company or primary care provider directly.
If you want to be seen by an Alliance surgeon, please ask your
primary care provider for a referral. Regardless of where you go
for surgery, you should feel comfortable with your care; all of
our surgeons give second opinions. If you have any questions about
the referral, please call us at 617-665-2555.
What to bring with
you to your appointment
Please plan to arrive 15 minutes early for your first visit to
one of our surgeons. This will give you time to register and fill
out all necessary paperwork. Patients should also bring the following
with them if possible:
- Any X-rays or other relevant diagnostic tests
- Your medical record that relates to your visit
- Current list of medications and dosages that you are taking
- Name and phone number of your Primary Care Physician
- Health Insurance information
- Co-payments not covered by insurance are expected at the time
of service
If you need to cancel your appointment for any reason, please call
us at 617-665-2555 as soon as possible so we can reschedule.
