TRANSITIONAL CARE
Transitional Care is an intermediate stage of care
for patients who do not need acute care but are not ready to go
home or be discharged to a long-term care facility. It is an excellent
option for patients who need more recovery time after illness, surgery,
or trauma.
Here at Cambridge Health Alliance, we are proud to
offer a Transitional Care Unit at our Somerville Hospital Campus.
This unit offers comfortable surroundings that allow patients to
continue their recovery within the hospital. This is beneficial
on two levels:
- Ongoing care can be provided by a medical team - doctors, physical
therapists, respiratory therapists, and case managers - who are
familiar with the patient
- Higher levels of medical care are close by in case they are
needed
